Sealed I-693 envelopes are sometimes lost in transit, misplaced by the recipient, or requested again by your attorney or USCIS. For $40 per person, added at the time of your original appointment, we will issue replacement sealed envelopes at no further charge.

What qualifies

We issue a replacement when you provide one of the following:

• A USCIS RFE or other written request for the I-693
• A written resubmission request from your attorney or law firm
• Written confirmation that the envelope was lost, damaged, or never received

What this does not cover

This plan covers reprinting your existing I-693 only. It does not cover corrections, changes to your information, additional testing, updated vaccinations, or a new examination. Any of those require a new I-693 with new dates and is billed separately.

Replacement envelopes reproduce the original examination and carry the original I-693 date.

No. Effective September 2025, all the lab tests for immigration medical must to be ordered by the civil surgeon as per CDC and USCIS. Not doing so will result in rejection of I-693 application!

When acceptable vaccination records are unavailable, there may be two options depending on the vaccine:

1. Receive an additional required dose, or
2. Provide acceptable laboratory evidence of immunity when permitted.

Laboratory evidence of immunity may be accepted for MMR, Hepatitis B, Varicella, and certain other vaccines when the testing meets current CDC requirements.

For polio, immunity testing is acceptable only when the test documents antibodies to all three poliovirus types. Because qualifying testing may not always be readily available, vaccination may be the simpler option.

Tetanus/Tdap vaccination requirements are generally satisfied through acceptable vaccination documentation or an appropriate vaccine dose rather than immunity testing.

If you are unsure, do not repeat vaccines unnecessarily before we review your records.

One dose is enough as long as it is not time for the second dose prior to your appointment. Second dose of Polio vaccine, Hepatitis B vaccine and Varicella vaccine is due 4 weeks after the first dose and hence, you will need a second dose of these vaccines if more than 4 weeks have been elapsed at the time of your appointment with the doctor since your first dose.

Any of the above laboratory testing which is required to be issued from our office, we will send you a lab order for you to complete testing at an independent CLIA-certified laboratory (Quest Diagnostics or LabCorp).

The laboratory will collect your sample and send results directly to our office. We will notify you once your results are reviewed.

A statement or certificate from another doctor regarding your childhood vaccinations, in the absence of the original vaccination records, may be considered fraudulent by USCIS. We kindly request that you provide the original vaccination records. If you do not have them, you may either receive an additional dose of the vaccine or provide proof of immunity by lab tests (lab tests for immunity are applicable only for MMR, Hepatitis B and Varicella).

We do not bill insurance directly for the I-693 immigration medical examination. Immigration medical exams are not covered under any insurance plans. However, you can submit the receipts for the fee paid to us your insurance to see if they will cover or not. Everyone can go to any pharmacy to get the vaccines using their insurance at minimal to no cost. We don't sell vaccines.
